About the role
Amity Health is seeking a dedicated Aboriginal Outreach Worker to help support Aboriginal and Torres Strait Islander people to better access and engage with available health care services.
In this role, you’ll assist clients to attend appointments, navigate the health system and maintain connection with primary and specialist services, contributing to improved uptake of care and the overall outcomes of the Integrated Team Care program.
Program Overview
The Integrated Team Care (ITC) program is funded by the WA Primary Health Alliance and delivered by Amity Health across the Great Southern, Coastal and Southern Wheatbelt regions.
The program supports Aboriginal and Torres Strait Islander people who are enrolled in chronic disease management with a General Practitioner and are living with, or at risk of developing, cancer, cardiovascular disease, chronic kidney disease, chronic respiratory disease or diabetes.
ITC provides coordinated support in partnership with General Practitioners to strengthen management plans and improve access to specialist services and approved medical aids and equipment, with the aim of improving long-term health outcomes.

This is a fixed-term, part-time position, 2 days per week based in Albany until 30 June 2027.
