About the role
Just for u Care
Just for u Care is a trusted provider of in-home care services, supporting older Australians and people with disability to live safely and independently at home. We are committed to dignity, respect, and delivering high-quality care for every client, every day.
We are currently seeking a full-time Care Partner to provide compassionate, person-centred care and support to our clients in their homes and communities. You will play a key role in helping clients maintain independence, wellbeing, and connection with their community.
About the Role
As a Care Partner, you will:
Join a dynamic team of Care Partners with diverse professional backgrounds
Provide high-quality, person-centred care for a caseload of clients in line with the Aged Care Quality Standards
Work from our Cheltenham office, with flexibility to visit clients across the Melbourne Metropolitan area, as well as Geelong, the Bellarine Peninsula, and the Mornington Peninsula.
Build and maintain care plans, monitor client wellbeing, and liaise with internal and external stakeholders to ensure the best outcomes
Key Duties and Responsibilities
Provide end-to-end support for clients receiving Support at Home Packages, including assessment and commencement
Work with clients to identify strengths and abilities and deliver appropriate care and services
Develop, review, and update care plans to support client goals
Liaise with medical, allied health, and community stakeholders
Maintain regular client contact, including monthly monitoring calls
Implement and manage client budgets
Complete administrative tasks, including case notes
Ensure compliance with Aged Care Quality Standards
Skills and Experience
Qualifications or experience in Nursing is essential
Experience in Case Management is preferred
Strong time management and organisational skills
Excellent verbal and written communication
Ability to build positive relationships with clients, families, and advocates
Ability to work autonomously and in a team environment
Current Police Check, driver’s licence, and reliable vehicle
Multitasking skills
Speaking a second language is an advantage
