About the Role

As Clinical Care Coordinator, you will play a pivotal role in leading our nursing and care team to ensure the consistent delivery of safe, effective, and compassionate care.

Key Responsibilities:

• Lead the planning, delivery, and evaluation of clinical care

• Mentor and support clinical staff in daily practice

• Promote continuous improvement across the care team

• Monitor staff practices and support professional development

• Manage incidents and escalate appropriately

• Facilitate case conferences and multidisciplinary communication

• Oversee specialised nursing care and therapeutic drug administration

• Ensure compliance with Aged Care Standards and regulatory requirements

About You

You are a compassionate and supportive professional who lives our core values of Trust Recognition Integrity and Acceptance.

Essential Criteria:

• Current AHPRA registration as a Registered Nurse

• Minimum 3 years post-registration experience with strong clinical knowledge of ageing-related issues

• Proven ability to coordinate and deliver high-quality care

• Sound understanding of Aged Care Quality Standards and relevant legislation

• Strong leadership, communication, and critical thinking skills

• Proficiency in MS Office and clinical software

• Commitment to ongoing professional development
