About the Role
As Clinical Care Coordinator, you will play a pivotal role in leading our nursing and care team to ensure the consistent delivery of safe, effective, and compassionate care.
Key Responsibilities:
Lead the planning, delivery, and evaluation of clinical care
Mentor and support clinical staff in daily practice
Promote continuous improvement across the care team
Monitor staff practices and support professional development
Manage incidents and escalate appropriately
Facilitate case conferences and multidisciplinary communication
Oversee specialised nursing care and therapeutic drug administration
Ensure compliance with Aged Care Standards and regulatory requirements
About You
You are a compassionate and supportive professional who lives our core values of Trust Recognition Integrity and Acceptance.
Essential Criteria:
Current AHPRA registration as a Registered Nurse
Minimum 3 years post-registration experience with strong clinical knowledge of ageing-related issues
Proven ability to coordinate and deliver high-quality care
Sound understanding of Aged Care Quality Standards and relevant legislation
Strong leadership, communication, and critical thinking skills
Proficiency in MS Office and clinical software
Commitment to ongoing professional development
