The Role

The Mobile Assessment Treatment Team (MATT) Case Manager will participate as part of a multidisciplinary team in providing comprehensive, specialist and early intervention mental health services to young people aged 12-25 years old who are referred to the program. 

As part of the multidisciplinary team, the Case Manager will provide comprehensive mental health assessment and acute home-based intervention to young people who are newly referred to the service, as well as the provision of crisis intervention and community treatment to current young people treated within the service.

Case Managers will be required to work on a rotating roster across extended hours including evenings, weekends and public holidays, and participate in on-call system.

The role involves the provision of highly skilled comprehensive; evidence based clinical case management and specialist psychological interventions consistent with the EPPIC model of care within the Service. The overall aims of case-management within the team are to:

·       Intervene early and effectively with young people experiencing a first episode of psychosis and who may also present with a range of co-morbid mental health issues.

·       To reduce the duration of untreated psychosis or at ultra-high risk (UHR) to minimise disability, adverse health and social impacts

·       To enable both symptomatic and functional recovery from a first episode of psychosis through the provision of evidence-based psychological therapy, home based assertive outreach, family work and skilled, youth friendly case management

Duties and Responsibilities of the Position

·       Consultation, liaison and linkage with internal and external service systems to provide a full range of tailored interventions for young people with a first episode of psychosis or at ultra-high risk.

·       Participate in the identification, reporting and management of generic and clinical risks

·       Maintain the rights and safety of patients by providing an advocacy role.

·       Adheres to the Mental Health Act, all headspace and Black Swan Health Policies and Procedures including Equal Employment Opportunity, Occupational Health and Safety, the Code of Conduct and other relevant legislation and policies.

·       Performs duties in accordance with the relevant Occupational Safety and Health and Equal Opportunity Legislation and Code of Conduct.

·       Engage in the debrief provided after any incident affecting the individual or team

·       Ensure that service delivery complies with legislative requirements and the policies and procedures of Black Swan Health and the program.

·       Work with the Team Coordinator to respond to concerns regarding child abuse or neglect in consultation

·       Ensure delivery of high-quality care based on existing evidence, standards and guidelines

·       With the Team Coordinator and other Senior Clinical staff, ensure clinical processes are effectively managed, including allocation of young people, critical incident responses, clinical review, discharges, business and team meetings

·       Develop, implement and evaluate appropriate treatment plans for each young person and their family in collaboration with young people, families and members of the multidisciplinary team

·       Practice according to the philosophy, standards, guidelines and protocols of the service and any other duties as required

·       Support the development of comprehensive mental health and dual diagnosis assessment and treatment, develop formulations and individual service plans for clients, risk management and delivery of individual, family and systemic treatment interventions

·       Provision of evidence based psychosocial therapeutic interventions that promote recovery for the young person and their family/significant others

·       Crisis response and home-based assertive outreach as required for young people engaged with the continuing care teams, to facilitate engagement and treatment

·       Other duties as required

The successful candidate will have

·       Current driver’s licence

·       Working hours flexibility including rostered on weekends and public holidays

·       Eligible to work in Australia

·       National Police Clearance (not older than 6 months)

·       Working with Children Check

·       Relevant tertiary qualifications and registration with AHPRA or membership with an appropriate professional body.

·       Experience in the application of evidence based clinical practice in the complex youth mental health field.

·       Experience in the development and coordination of individualised psychosocial interventions.

·       Proven ability to act autonomously as well as work collaboratively with other Clinicians in highly complex and acute settings.

·       Highly developed interpersonal and communication skills including ability to articulate risk, symptomology, ethology and care plans succinctly and within an evidence-based formulation model.

·       Knowledge of relevant legislation such as the Mental Health Act including proposed reforms and the Children, Youth and Families Act and any other relevant legislation

·       Demonstrated experience to engage with complex, vulnerable and/or marginalised young people, their families and /or significant others.

·       Demonstrated experience working within acute crisis situations

·       Experience in the application of evidence based clinical practice in first episode psychosis (Highly desirable)
