Female Disability Support Worker (Melbourne West and North)

About the role

Matrix Healthcare is seeking compassionate and reliable Disability Support Workers to join our team. The role requires excellent English communication skills, a strong understanding of the NDIS environment, and the ability to build meaningful relationships with clients. Experience in personal care and supporting individuals with diverse needs is highly regarded.

Key responsibilities

• Providing individualized care and support to clients with mental health disabilities
• Assisting clients with daily living activities, including personal care, meal preparation, and household tasks
• Accompanying clients to appointments, social activities, and community engagement
• Developing and implementing personalized support plans in collaboration with clients and their families
• Monitoring clients' progress and providing regular feedback to the clinical team
• Advocating for clients' rights and ensuring their needs are met
• Maintaining accurate client records and adhering to all relevant policies and procedures

About you

• Disability support worker with excellent English speaking ability
• Relevant qualifications in disability support, mental health, or a related field
• Extensive experience working with people with mental health disabilities, preferably in a community-based setting
• Strong communication and interpersonal skills, with the ability to build rapport and trust with clients
• Excellent problem-solving and critical thinking skills, with a focus on person-centered care
• Demonstrated commitment to empowering and supporting clients to achieve their goals
• A valid driver's license and access to a reliable vehicle
• A current Police Check and Working with Children Check
