Amity Health are seeking a compassionate and proactive care finders to join our highly regarded team as we expand our services to the, Wheatbelt (Narrogin), Goldfields (Kalgoorlie Boulder), and Southern Goldfields (Esperance) regions.
In this role, you will provide intensive, person-centred support to older people who experience significant barriers to accessing aged care and community services. You will walk alongside clients, helping them navigate complex systems, connect with community supports, and advocate for our clients needs – ensuring no one is left behind.
About Care Finder
On behalf of the WA Primary Health Alliance and the Commonwealth Government, Amity Health delivers the care finder program. A specialised, face-to-face support service which helps vulnerable older people who cannot navigate the aged care system on their own. Care finders work alongside clients to understand their needs, explain care options, complete My Aged Care processes, connect them with assessments and services, and resolve barriers that may prevent access. The service focuses on people who are isolated, experiencing disadvantage, or facing complex challenges, ensuring they receive personalised, intensive, and trusted support to access the care they need.
