Amity Health are seeking a compassionate and proactive care finders to join our highly regarded team as we expand our services to the, Wheatbelt (Narrogin), Goldfields (Kalgoorlie Boulder), and Southern Goldfields (Esperance) regions. 

In this role, you will provide intensive, person-centred support to older people who experience significant barriers to accessing aged care and community services. You will walk alongside clients, helping them navigate complex systems, connect with community supports, and advocate for our clients needs – ensuring no one is left behind. 

About Care Finder 

On behalf of the WA Primary Health Alliance and the Commonwealth Government, Amity Health delivers the care finder program. A specialised, face-to-face support service which helps vulnerable older people who cannot navigate the aged care system on their own. Care finders work alongside clients to understand their needs, explain care options, complete My Aged Care processes, connect them with assessments and services, and resolve barriers that may prevent access. The service focuses on people who are isolated, experiencing disadvantage, or facing complex challenges, ensuring they receive personalised, intensive, and trusted support to access the care they need.  

About the Role 

This is a highly rewarding and hands-on role where you will make a genuine difference to clients  your community. As a care finder, you will: 

• Deliver assertive outreach, meeting clients in their homes or community settings to build trust and engagement  
• Provide end-to-end support, assisting clients to navigate My Aged Care, attend assessments, and connect with appropriate services  
• Work with individuals experiencing complex and overlapping needs, including mental health, housing instability, and social disadvantage  
• Help clients understand their options and make informed decisions about their care and support pathways  
• Advocate on behalf of clients, addressing barriers and escalating risks or unmet needs 
• Develop and implement care plans, supporting clients through to successful service connection 
• Build strong relationships with aged care providers, GPs, hospitals, and community organisations to ensure coordinated care  
• Manage a diverse and active caseload, adapting your approach to meet individual needs 
• Maintain accurate, timely documentation and contribute to quality improvement and reporting requirements  

This is a part time, 3 day per week role, based in Narrogin, WA. This role may involve regular travel across regional communities and requires flexibility to meet client needs. 

About You 

You are an experienced, compassionate, resilient, and adaptable practitioner who delivers a person-centred, strengths-based and trauma-informed approach to care.  You bring: 

• Significant experience in aged care, community services, health, mental health, or social and community settings. 
• Strong understanding of system navigation, social and health barriers faced by vulnerable or disadvantaged individuals.  
• Demonstrated ability to engage and build trust with hard-to-reach or socially isolated clients. 
• Experience managing complex cases and coordinating across multiple service systems. 
• Excellent communication and interpersonal skills, including advocacy and relationship-building 
• Ability to work independently while collaborating with multidisciplinary teams. 
• Strong organisational skills and confidence managing complex caseloads. 

Essential Requirements: 

•  Tertiary qualifications, degree –level in Social Work, Community Services (Disability or Aged Care), or relevant industry with relevant experience.  
• Current National Police Clearance and WA Driver’s License  

Desirable: 

• Mental Health First Aid or similar training 
• Experience working with First Nations communities, CALD populations, or people experiencing homelessness or disadvantage
