Are you an experienced Registered Nurse looking to make a genuine difference in the lives of older Australians?
CMHC SSW is seeking a passionate and experienced Clinical Nurse to join our growing team delivering high-quality clinical care through the Australian Government's Support at Home program.
Based in Fairfield, you will support older people living throughout Fairfield and the surrounding suburbs, helping them remain safe, healthy and independent in their own homes.
Working alongside our Care Manager and leadership team, you will play a pivotal role in clinical governance, complex care management, staff education, quality improvement and the ongoing delivery of person-centred community care.

As our Clinical Nurse, you will provide expert clinical leadership across our community services, ensuring care is evidence-based, compliant with legislative and professional standards, and tailored to each client's individual goals and needs.
Working across Fairfield and surrounding suburbs, you will collaborate with Care Managers, General Practitioners, Allied Health Professionals, hospitals and families to coordinate high-quality clinical services that support ageing in place under the Support at Home program.
This position includes participation in a shared after-hours clinical on-call roster.
Provide clinical leadership and expert nursing advice across the organisation.
Support Registered Nurses, Enrolled Nurses and Support Workers with complex clinical care.
Conduct comprehensive nursing assessments and clinical reviews.
Develop, implement and evaluate individualised care plans.
Monitor client outcomes and adjust clinical interventions as required.
Support the safe management of clients with complex health needs.
Deliver clinical services under the Australian Government's Support at Home program.
Complete comprehensive nursing assessments that support independence, wellness and reablement.
Work collaboratively with Care Managers to ensure services align with assessed needs and client goals.
Liaise with My Aged Care, General Practitioners, Nurse Practitioners, Allied Health Professionals, hospitals and other external providers.
Promote preventative healthcare and evidence-based community nursing practices.
Lead and contribute to the organisation's Clinical Governance Framework.
Monitor compliance with the Aged Care Quality Standards and relevant legislation.
Lead clinical audits, documentation reviews and quality improvement initiatives.
Investigate clinical incidents and implement corrective actions.
Monitor clinical risks and promote a culture of continuous improvement.
Lead infection prevention and control activities across the organisation.
Develop and review infection control policies and procedures.
Conduct infection prevention audits.
Provide education regarding PPE, hand hygiene and standard precautions.
Support outbreak management and quality improvement initiatives.
Deliver clinical education and competency-based training to nursing and care staff.
Mentor and support Registered Nurses, Enrolled Nurses and Support Workers.
Coordinate annual competency assessments.
Deliver education in medication management, wound care, diabetes management, continence care, catheter management, enteral feeding, palliative care, pressure injury prevention, infection prevention and control, clinical documentation and recognition of deteriorating clients.
Investigate clinical incidents and adverse events.
Undertake root cause analysis where appropriate.
Identify trends and implement preventative strategies.
Participate in organisational clinical risk management.
Ensure mandatory reporting obligations are met.
To be successful in this role you will have:
Current unrestricted AHPRA registration as a Registered Nurse.
Minimum two years' post-registration nursing experience.
Experience in community nursing, aged care, home care or primary health.
Strong clinical assessment and care planning skills.
Knowledge of clinical governance and quality improvement.
Excellent leadership, communication and mentoring skills.
Current Australian Driver Licence and access to a reliable motor vehicle.
Current First Aid and CPR certification.
National Police Check.
NDIS Worker Screening Check (where applicable).
Postgraduate qualifications in Community Nursing, Gerontology, Primary Health Care or Clinical Leadership.
Experience delivering services under the Support at Home, Home Care Packages, DVA Community Nursing or NDIS programs.
Experience in accreditation, infection prevention and quality systems.
Ability to speak and communicate effectively in Mandarin and/or Cantonese is highly desirable to support our diverse client community.
