Intake, Triage and Assessment Worker

Summary:

The Intake, Triage and Assessment Worker is responsible for delivering comprehensive intake, psychosocial assessment, and triage services across all Eleanor Duncan Aboriginal Services programs. The role provides culturally responsive assessment, risk identification, supported decision-making, and short-term case coordination (up to 12 weeks) while facilitating internal and external referrals to ensure the most appropriate and timely outcomes for clients. Working within a transdisciplinary model, this role collaborates with the Intake and Triage Nurse and broader program teams to ensure holistic, culturally safe and person-centred service allocation.

Responsibilities:

• Conduct culturally responsive intake, psychosocial and risk assessments across all EDAS programs.
• Undertake suicide, self-harm, DFV and child protection risk assessments and develop safety plans within scope.
• Hold and coordinate a short-term caseload (up to 12 weeks) while facilitating appropriate referral and allocation.
• Provide supported decision-making to clients to navigate health, social, justice and community systems.
• Participate as a decision-maker in transdisciplinary intake, referral and allocation meetings.
• Collaborate closely with the Intake & Triage Nurse and program teams to determine urgency and service pathways.
• Exercise professional judgement and escalate complex or high-risk matters as required.
• Prepare high-quality case notes, referral documentation and risk documentation within required timeframes.
• Advocate for clients within internal and external service systems.
• Mentor junior intake staff as required and contribute to shared practice development.
• Identify systemic service gaps and contribute to service improvement discussions when requested.
• Maintain strong knowledge of community, government and non-government referral pathways.
• Represent Gulayi in internal case reviews and external interagency meetings when required.
• Uphold trauma-informed, strengths-based and culturally safe practice at all times.
• Maintain accurate data entry and reporting in client management systems.
• Contribute to continuous quality improvement processes within the intake function.         

Qualifications:

• Bachelor of Social Work or related discipline, or significant relevant professional experience Demonstrated lived experience recognised as a genuine qualification
• Current NSW Driver’s Licence
• Current NSW Working with Children Check
• Satisfactory Police Check – no serious/criminal court record
• Demonstrated experience conducting psychosocial and risk assessments
• Ability to undertake suicide, DFV and child protection risk assessment and safety planning within scope
• Strong communication and interpersonal skills, especially with Aboriginal clients and families
• Demonstrated cultural capability and ability to work in a culturally safe and respectful manner
• Ability to work autonomously while contributing to transdisciplinary decision-making

• Understanding of the impact of intergenerational trauma, racism, and social determinants on Aboriginal health
• Awareness of relevant policies, frameworks, and legislation (e.g. Closing the Gap, Mental Health Act, privacy laws)
• Understanding of strengths-based, person-centred, and culturally safe practice models
• Local knowledge of community-based and mainstream supports, housing, and employment services
• Engagement with multi-disciplinary teams, including LHDs, NGOs, and primary care
• Experience delivering culturally responsive support and advocacy
