About the role

We are seeking a Registered Nurse (RN) / Care Partner to join our team in Willetton, WA on a full-time basis. This is a pivotal position within our aged care services, where you will play a crucial role in delivering nursing care and personal support to our clients in their homes. You will work collaboratively with our multidisciplinary team to ensure clients receive safe, respectful and individualised care that promotes their wellbeing and independence. This role requires a flexible approach, as you will be providing care across multiple client locations within the local area.

Key responsibilities

• Provide direct nursing care and personal assistance to aged care clients in their home environment, including wound care, medication administration and health monitoring

• Conduct comprehensive health assessments and develop individualised care plans in consultation with clients, families and healthcare professionals

• Monitor clients' health status and vital signs, reporting any changes or concerns to appropriate medical professionals

• Assist clients with activities of daily living, including personal hygiene, mobility support and nutritional needs

• Maintain accurate and detailed client records and documentation in accordance with aged care standards and regulatory requirements

• Communicate effectively with clients, families and team members to ensure continuity of care and address any concerns

• Support clients with medication management and provide health education to promote self-care and wellbeing

• Work collaboratively with other care team members and external healthcare providers to deliver holistic care

• Respond compassionately to clients' emotional and social needs, supporting their independence and quality of life

What we're looking for

• Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA)

• Demonstrated experience in aged care nursing or community health nursing

• Strong knowledge of aged care standards, regulations and best practice guidelines

• Excellent clinical skills, including wound care, medication administration and health assessment

• Ability to work independently and make sound clinical decisions whilst maintaining communication with the wider care team

• Compassionate and person-centred approach to care delivery

• Strong communication and interpersonal skills with the ability to build trust with clients and families

• Flexibility to work across multiple client locations and adapt to varied care environments

• Current First Aid certification and CPR qualification (or willingness to obtain)

• Valid driver's licence and access to reliable transport for home visits

• Experience with electronic health records and aged care documentation systems is advantageous
