About the Role

You'll support adults with chronic and complex health needs to build independence and improve their quality of life through practical, person-centred care.

You'll partner with clients, carers and other health professionals to deliver evidence-based services in home and community settings. This includes:

• Completing comprehensive assessments, care planning and home visits
• Recommending assistive technology, equipment and home modifications
• Supporting clients to navigate aged care and community funding pathways
• Delivering education, coaching and self-management strategies
• Collaborating with a multidisciplinary allied health team
• Participating in supervision, case reviews and continuous improvement initiatives
• Maintaining high-quality documentation and contribute to service outcomes

About You

You're motivated and enjoys building relationships, working collaboratively and helping people achieve what matters most to them.

You'll bring:

• Current AHPRA registration
• Experience or knowledge in functional assessments, home modifications and assistive technology
• Strong communication, organisation and problem-solving skills
• The ability to work effectively with people from diverse cultural and linguistic backgrounds
• A commitment to evidence-based practice and high-quality client care
