Southern Cross Care QLD has an exciting opportunity for a proactive and self-directed full-time Care Partner to join our Gold Coast Community team working from the Ashmore office.

About the role

In this dynamic role, based at Ashmore, you will work closely with older people living independently in the community, supporting them to achieve their goals and remain safely in their own homes.

You will manage a caseload of Support at Home (SAH) and support Commonwealth Home Support Programme (CHSP) clients. Working collaboratively with clients, families, health professionals and service providers, you will coordinate high-quality, person-centred care that promotes independence, choice and wellbeing.

This is a rewarding role for someone who enjoys building relationships, solving problems and advocating for positive client outcomes.


Some of your responsibilities will include:

• Developing personalised care plans in collaboration with clients and their families where appropriate.
• Developing budgets with clients, utilising funding to meet their needs effectively.
• Planning and coordinating the delivery of scheduled services such as personal care, meal preparation, cleaning, and transportation.
• Monitoring the implementation of care plans and adjusting as needed.
• Monitoring clients funding, taking corrective action to ensure clients services are provided within funding and support the management of any unspent funds.
• Ensuring CHSP services are provided in accordance with CHSP guidelines.
• Promoting clients' rights, preferences and independent decision making.
• Conducting regular reviews and reassessments as needed to ensure that clients' needs are being met effectively.
• Providing information and support to clients and their families regarding available services and resources.
• Liaising with healthcare professionals, service providers, and community organisations to ensure coordinated care.
• Maintaining accurate records of Care management services, and documenting client interactions and service provision.

What will you bring to the role?

• Certificate IV in Community Services or equivalent.
• Experience in a Care Partner, Case Manager, Care Coordinator or similar community aged care role.
• Excellent assessment, care planning and case management skills.
• Demonstrated knowledge and experience in delivering services under Support at Home - formerly Home Care Package Program.
• Demonstrated knowledge and experience in delivering services under the Commonwealth Home Support Program (CHSP).
• Demonstrated knowledge and experience of working in accordance with Aged Care Standards and legislation.
• Practical approach with realistic and sensible mindset.
• High attention to detail, ensuring accuracy and thoroughness.
• Ability to follow processes and creating and implementing efficient work practices.
• Ability to manage time efficiently and prioritise responsibilities.
• Excellent communication skills at all levels.
