About the role
About the role
As a Disability Support Worker with YSCS, you will provide person-centred support to NDIS participants within Supported Independent Living (SIL), community and individual support environments.
Key responsibilities
Supporting participants with daily living and personal care
Assisting with community access, appointments and social activities
Supporting participants to develop independence and achieve their individual goals
Medication assistance in accordance with competency, policy and participant requirements
Meal preparation and household activities
Following Behaviour Support Plans, health plans and participant-specific support strategies
Completing accurate, objective and participant-focused progress notes
Identifying and reporting incidents, risks, safeguarding concerns and changes in participant wellbeing
Working collaboratively with families, guardians, allied health professionals and the broader YSCS team
Supporting participants with dignity, respect and professional boundaries
About you
Dependable, compassionate and professional
Understand that working in disability support carries significant responsibility for the safety, rights and wellbeing of the people we support
Previous disability or community-services experience is highly regarded, particularly experience supporting people with complex needs, behaviours of concern, mental health needs or SIL supports
NDIS Worker Screening Check
Working with Children Check where applicable
Police Check no older than 6 months
Current First Aid and CPR
Valid Australian driver's licence
Reliable transport
Relevant NDIS Worker Orientation training
Appropriate work rights in Australia
Certificate IV in Individual Support, Disability, Community Services or a related qualification is highly regarded
Ability to work a flexible roster, including mornings, afternoons, weekends, sleepovers and active overnight shifts where required
