About The Role

The role is to manage a case load of clients under CHSP/HCP approved funding. Maintaining budgets, on boarding growth of new clients and managing care requirements to maintain clients independency.

What You Bring

Evidence of experience in a senior case manager roleRelevant tertiary qualification in the community sector will be desirable Demonstrated experience in the aged/community/disability sector particularly working with clients with complex needsFull Drivers Licence and own reliable registered and insured Vehicle Smart PhoneKnowledge of My Aged Care portal/assessment process Willing to work in a team environment A desire to join a team committed to the care of others in a mission based organisationNDIS clearance (or willingness to obtain one)Police clearance (within the last 6 months Or willingness to obtain one)
