About the role
What you’ll be doing
The Outreach Worker engages with people experiencing homelessness, providing tailored case management, coordinating referrals to essential services, advocating for clients, and collaborating with the team to support individuals in achieving and maintaining independent tenancies.. The role also involves developing support plans, maintaining records, and contributing to effective service delivery.
Engage with individuals experiencing homelessness and provide tailored case management.
Coordinate referrals for housing, healthcare, employment, and other essential services.
Advocate for clients and help them overcome barriers to maintaining independent tenancies.
Develop and implement support plans, maintain diligent records, and ensure confidentiality.
Collaborate with team members and participate in regular meetings to deliver effective services.
Support the wider team and organization as needed.
What we’re looking for
Experience in case management or support services.
Understanding of the needs of people experiencing homelessness or housing crisis.
Relevant qualifications and/or experience in human services.
First Aid certificate (or willingness to obtain).
Satisfactory Police, NDIS, and Working with Children checks.
Current driver’s licence.
Strong client focus, proactive approach, and effective communication skills.
Ability to build positive relationships and resolve conflict constructively.
Commitment to integrity, confidentiality, and organizational policies.

This role works a rotating weekday schedule:
Monday, Tuesday, and Thursday - 8:00am to 4:00pm
Wednesday and Friday - 7:00am to 3:00pm.
