What You'll Be Doing

Assess, plan, implement and evaluate community nursing care, in the patient home or clinic, in collaboration with individual/s and the greater health care team to achieve quality patient outcomes.

Candidates Will Need To Meet The Following Criteria

Registered Nurse –Division 1 with Australian Health Practitioner Regulation Agency (AHPRA); and Current NSW Drivers Licence.Demonstrated ability to communicate effectively with patients, carers and a diverse range of health and non-health care providers.Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, patient safety, quality of care and team cohesion.
