Spinal Life Australia is a not-for-profit organisation passionate in our commitment to rebuilding lives, promoting and celebrating independence, and preventing spinal injuries. Our Personal Support & Home Care Services team support people across Perth with spinal cord injury and other physical disabilities so that they can live the life they choose.
We are seeking a Registered Nurse to join us as a Clinical Care Coordinator to coordinate high quality, person-centred services for our clients[KB1] . The role provides day to day supervision of Support Workers, contributes to workforce planning and roster management, and ensures services are delivered safely and consistently. Acting as a central point of coordination, the Clinical Care Coordinator works closely with clients, families, support workers, practitioners and external stakeholders to translate client goals and practitioner recommendations into practical, measurable and achievable support outcomes.
Key Responsibilities Will Include
Provide clinical leadership and advice to management team and rostering team of client clinical needs.Formally review client care plans to ensure compliance to standards of care for quality review.Ensure individualised quality service is provided and maintained to our clients receiving personal care and support services.Management of relationships with internal and external stakeholders that ensures individual service delivery outcomes.[KB2] Effective leadership of a team of personal support workers, to ensure delivery of great quality services to our clients.Monitor client funding packages to agreed budgets and implement strategies to address any ongoing issues where required.Partner and collaborate with our clients and stakeholders to deliver appropriate and sustainable service. Travel will be required with regular local visits by car and travel to remote and regional areas, by air.

About You
Experience as a Registered NurseExperience in community services, or the disability sector is highly regarded Ability to work within a quality management and compliance framework Strong organisational skills, with the ability to juggle multiple priorities while delivering excellent client outcomes Proven experience leading, supervising, mentoring and developing staff Advanced communication skills, with the ability to juggle priorities while prioritising client outcomes Excellent skills at building effective stakeholder relationships, acting as a positive representative for the organisation and your clients. Experience in or the ability to rapidly acquire knowledge, working within a quality management framework;A current Australian driver’s licence and a reliable vehicle.A current NDIS Workers Screening Check and a Working with Children Check, or be willing to apply for one.
