Aboriginal Health Worker

About the Role

As an Aboriginal Health Worker within our Social and Emotional Wellbeing (SEWB) team, you will provide culturally safe, holistic support to Aboriginal individuals, families, and community members experiencing a range of challenges that impact their health and wellbeing.

Working within the yadhaba  program, you will support clients to strengthen their social and emotional wellbeing through case management, counselling support, care coordination, advocacy, and community education. You will work closely with families, community members, internal teams, and external service providers to ensure clients can access the supports they need to achieve positive outcomes.

This varied role includes:

• Conducting intake assessments and developing case management plans
• Providing counselling, support, advocacy, and referrals for clients experiencing mental health and AOD challenges
• Supporting families affected by mental health and substance use concerns
• Coordinating care and working collaboratively with multidisciplinary teams
• Building strong relationships with community members and service providers
• Delivering community education sessions, yarning circles, and wellbeing workshops
• Maintaining accurate client records and contributing to program reporting
• Supporting a culturally safe environment that promotes healing, resilience, and self-determination

Key Responsibilities

• Support clients with mental health, social and emotional wellbeing, alcohol and other drug (AOD), housing, and other psychosocial needs.
• Provide culturally safe case management, care coordination, advocacy, referrals, and client support.
• Conduct client assessments and develop individual case management plans in partnership with clients.
• Provide nontherapeutic counselling and support to Aboriginal clients and families affected by mental health and AOD concerns.
• Build strong partnerships and work collaboratively with both internal and external services to improve client outcomes.
• Liaise with Aboriginal Community Controlled Organisations, government agencies, health services, housing providers, and other community organisations to coordinate support.
• Facilitate groups, workshops, yarning circles, community education sessions, and health promotion activities for adults.
• Support clients to access health, housing, counselling, welfare, and other services that enhance their overall wellbeing.
• Work collaboratively with the SEWB and Primary Health Care teams to provide holistic, client-centred care.
• Maintain strong relationships within the local Aboriginal community to promote early intervention and referrals.
• Assist with community engagement activities and contribute to the development and delivery of culturally safe wellbeing programs.
• Maintain accurate client records, case notes, and reporting in accordance with organisational policies and funding requirements.
• Contribute to continuous quality improvement activities and uphold EDAS values, policies, and Code of Conduct.

What We’re Looking For

Essential Requirements

• Certificate IV in Mental Health, Alcohol & Other Drugs, Aboriginal Primary Health Care, Community Services, or equivalent experience (desirable)
• Experience working with Aboriginal individuals, families, and communities
• Demonstrated counselling, client assessment, and case management experience
• Strong knowledge of mental health and alcohol and other drug issues
• Experience building relationships with community members and external agencies
• Excellent communication and interpersonal skills
• Ability to work independently while contributing positively to a multidisciplinary team
• Current NSW Driver Licence
• Current NSW Working With Children Check
• Satisfactory National Police Check
• Strong computer and record-keeping skills

Desirable

• Knowledge of trauma-informed, culturally safe, and strengths-based practice
• Experience facilitating community education programs, workshops, or yarning circles
• Understanding of Closing the Gap priorities and Aboriginal health frameworks
• Experience working across health, housing, justice, and social support systems
• Advocacy, negotiation, and care coordination skills
