About the role
Leaving hospital and returning to everyday life can be a pivotal point in a person’s mental health journey. Having the right people alongside you can make all the difference – building confidence, strengthening wellbeing and creating connections that support a life beyond hospital. That’s where Hospital to Home (H2H) comes in. We’re looking for an experienced and values-driven Team Leader to lead the people, practice and partnerships that bring this important program to life.
This is a hands-on leadership role where you’ll be close to your team and the work they do. You’ll create an environment where employees feel supported to make good decisions, grow their capability and deliver thoughtful, high-quality support – including when situations are complex and the path forward isn’t always straightforward. You’ll also work beyond the boundaries of your team, building trusted relationships with Hospital and Health Services, community organisations and other partners to strengthen coordination and help create smoother, more connected pathways from hospital back into community life.

About you!
You’re a confident and compassionate people leader who knows how to bring out the best in a team while keeping quality, safety and the people we support at the centre of decision-making.
You’ll bring demonstrated leadership experience from mental health, community services or a hospital setting, along with a strong understanding of recovery-oriented, person-centred practice. You’re comfortable supervising and developing employees, working through complex situations and building trusted relationships with a range of stakeholders.
You’ll be someone who can see both the people and the bigger picture – organised enough to keep a busy service moving, confident enough to make sound decisions when challenges arise, and collaborative enough to bring people together around a shared outcome.
