Leaving hospital and returning to everyday life can be a pivotal point in a person’s mental health journey. Having the right people alongside you can make all the difference – building confidence, strengthening wellbeing and creating connections that support a life beyond hospital. That’s where Hospital to Home (H2H) comes in. We’re looking for an experienced and values-driven Team Leader to lead the people, practice and partnerships that bring this important program to life.

This is a hands-on leadership role where you’ll be close to your team and the work they do. You’ll create an environment where employees feel supported to make good decisions, grow their capability and deliver thoughtful, high-quality support – including when situations are complex and the path forward isn’t always straightforward. You’ll also work beyond the boundaries of your team, building trusted relationships with Hospital and Health Services, community organisations and other partners to strengthen coordination and help create smoother, more connected pathways from hospital back into community life.

What you’ll be doing

You’ll have the opportunity to:

• Lead a team with purpose – create an engaged, accountable and supportive team culture where people are equipped to do their best work.
• Grow capability and confidence – coach, supervise and develop employees, creating space for reflective practice, learning and professional growth.
• Guide quality practice – champion person-centred, strengths-based and trauma-informed approaches and provide guidance when situations require sound judgement and considered decision-making.
• Keep the service moving – coordinate service delivery, workloads and priorities while responding effectively to operational challenges, risks and emerging issues.
• Strengthen the connections around people – work collaboratively with participants, families, carers and service partners to support continuity and coordinated care.
• Build meaningful partnerships – develop strong relationships with Hospital and Health Services, community organisations and local networks to strengthen integrated service delivery.
• Make good services even better – use service insights, performance and feedback to identify opportunities, solve problems and continuously improve how H2H operates.
• Lead our values in action – bring Respect, Bravery, Compassion and Striving for Excellence to the way you lead, make decisions and support others.

About you!

You’re a confident and compassionate people leader who knows how to bring out the best in a team while keeping quality, safety and the people we support at the centre of decision-making.

You’ll bring demonstrated leadership experience from mental health, community services or a hospital setting, along with a strong understanding of recovery-oriented, person-centred practice. You’re comfortable supervising and developing employees, working through complex situations and building trusted relationships with a range of stakeholders.

You’ll be someone who can see both the people and the bigger picture – organised enough to keep a busy service moving, confident enough to make sound decisions when challenges arise, and collaborative enough to bring people together around a shared outcome.

You’ll also bring:

• Strong leadership, coaching and staff supervision skills, with the ability to build capability, accountability and engagement.
• Sound judgement and problem-solving skills, particularly when navigating complex or competing priorities.
• Strong communication and relationship-building skills, with the confidence to work collaboratively across hospital, community and service environments.
• An understanding of recovery-oriented, strengths-based and trauma-informed approaches to mental health support.
• Strong organisational skills and the ability to balance people leadership with the operational needs of the program.
• A relevant tertiary qualification in social work, psychology, human services or a related field, or equivalent relevant experience.
