About the role
Your Contribution
This position of Indigenous Health Worker (IHW) is responsible for the delivery of comprehensive primary health care services including sharing informative resources and imparting educational strategies to patients with a view to improving patient health outcomes. The position is an integral part of the multidisciplinary health care team, particularly in relation to supporting health team communication and advocating on behalf of patients both with internal and external stakeholders to ensure patients access the care and services they need. The position also involves, but is not limited to the following:
· Supporting patients in accessing culturally safe medical care within the clinic
· Performing clinical duties within the health worker scope of practice including health checks assessments, clinical observations such as blood pressure, performing clinical tests such as ECGs, micro-urines, and point of care testing such as HbA1c and INR
· Contributing to formulation and updates of patient care plans
· Providing patient education and community health promotion
· Maintaining clinical rooms to ensure these are fully stocked, checked, clean and tidied on a regular basis daily.
Duties & Responsibilities
The Indigenous Health Worker (IHW):
· Ensures patient interventions are carried out in an effective and timely manner
· Conducts a minimum of Five (5) Health Education and billing episodes (81300’s) per week
· Process a minimum of Five (5) Patient Education and billing (10950’s) episodes per week, recorded in the Best Practice system
· Ensures progress notes, reports and records are completed in the required format
· Follows policies and procedures relevant to the work area
· Pursues professional development opportunities and fulfils registration requirements
· Promotes and contributes to a safe, secure environment for staff, patients and visitors
· Complies with MBS billing requirements MBS Items
· Provides culturally safe and appropriate health care to meet the needs of the community
· Provides health management education to patients in consultation with patients
· Undertakes clinical assessments, tests and procedures within appropriate scope of practice
· Recognises limitations to scope of practice, calling for help and escalating care where appropriate
· Contributes to the effective formulation and updating of patient care plans
· Monitors patients’ health needs and outcomes
· Provides Cultural mentoring – support and guidance to non-Aboriginal/Torres Strait Islander colleagues on culturally safe and appropriate patient interactions
· Participates in staff professional development activities
· Fulfills registration requirements
· Ensures consult and treatment rooms are cleaned and stocked
· Follows infection control protocols
· Promotes a safe and harmonious environment for staff, patients, and visitors
