About the role
At My Community Health Sunshine Coast, we believe growing older shouldn't mean giving up independence, choice or quality of life. My Community Health is seeking talented and driven Community Occupational Therapist to join our established dedicated In-Home Nursing Team, servicing the Sunshine Coast regions.
As a part-time Community Occupational Therapist, you will play a vital role in providing comprehensive, high-quality services to the local community. With a focus on improving the overall health and well-being of our clients, this is an excellent opportunity to make a meaningful impact and contribute to the growth and success of our organisation. My Community Health supports client referrals from private, aged care and NDIS sectors.

You'll work closely with clients, families, care workers and health professionals to coordinate services, identify risks early, support changing care needs and ensure clients receive exceptional person-centred care.

Conduct thorough initial assessments to develop customised treatment plans for each client.
Deliver a range of physiotherapy interventions, including manual therapy, exercise prescription, and rehabilitation programs
Collaborate with clients, their families, and other healthcare professionals to ensure seamless, coordinated care
Maintain accurate and up-to-date client records, ensuring compliance with all relevant policies and regulations
Participate in regular team meetings and professional development opportunities to stay informed of industry best practices
Champion a client-centred approach, advocating for the unique needs and goals of each individual
Tertiary qualification in Occupational Therapist and current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
Demonstrated ability to effectively communicate and build strong relationships with clients, families, and multidisciplinary teams
Excellent clinical reasoning and problem-solving skills, with a commitment to evidence-based practice
Passion for promoting health, wellbeing, and independence in the local community
Flexible and adaptable approach, with the ability to work collaboratively in a team environment
Must have Australian working rights
