We’re looking for an experienced and compassionate Support Coordinator who is committed to achieving positive outcomes for participants and their families.

About the Role

This is a dedicated Support Coordination role focused on helping participants understand, implement and maximise the benefits of their NDIS plans. The role is centred on delivering high-quality, participant-focused support, building strong relationships, and assisting participants to achieve their goals and access the services they need.

What You’ll Be Doing

• Supporting participants to understand and implement their NDIS plans
• Coordinating services and connecting participants with suitable providers
• Building strong relationships with participants, families, and stakeholders
• Promoting independence, choice, and capacity building
• Monitoring budgets and ensuring NDIS compliance
• Preparing reports and supporting plan reviews
• Advocating for participant goals and positive outcomes

What You’ll Bring

• Experience in NDIS Support Coordination and/or case management
• Strong understanding of the NDIS and relevant frameworks
• Excellent communication and relationship-building skills
• Strong organisational and time management abilities
• Qualifications in Disability, Social Work, Psychology, Allied Health, or a related field (desirable)
• Confidence using Microsoft Office and case management systems

Essential Requirements

• NDIS Worker Screening Check
• Working With Children Check (WWCC)
• National Police Check
• Current Driver’s Licence and reliable vehicle
