Case Manager

Position Purpose

The Case Manager provides general case work and therapeutic intervention for children and young people,

their families, carers, within the permanency support program using contemporary practices.

This is an entry level role in case management for the Permanency Team.

Key Duties & Responsibilities

• Develop and implement case plans for children and young people in collaboration with relevant

stakeholders and provide ongoing therapeutic intervention and case management

• Engage with children, young people, families and carers, and undertake assessments to identify their

support needs

• Assist in the transition of children and young people entering, and exiting permanency support program

care arrangements

• Maintain accurate records and documentation in a computer-based case management system, in line

with the Office of Children’s Guardians practice standards, which may be used for the purposes of

Children’s Court proceedings

• Adhere to all the Family Spirit policy and procedures as well as the Department of Communities and

Justice (DCJ) polices and mandatory reporting requirements relevant to the role.

• Meet KPI’s case load of 7- 10 CYP, inclusive of monthly home visits, 1:4 Family Time visits, ability to

develop and review case plans.

• Network with other relevant stakeholders, as required

• Undertake the duties of the role while respecting cultural and religious practices

• Participating in an agency on call roster

• Undertake appropriate WHS training and comply with Family Spirit’s WHS policies and procedures. This

includes job specific requirements as well as minimising any potential risks and reporting any hazards or

incidents that you may observe or be involved in

• Any other duties as directed from time to time.

Knowledge, Skills and Experience

• A minimum of a Diploma in relevant discipline such as Community Services. Preference bachelor’s

degree in teaching, Social Science, Nursing, Counselling.

• Experience working with and providing quality assessment and case management services to at risk

families or children/young people in out-of-home care and carers.

• Demonstrated understanding of the principles of contemporary case management practice.

• High level oral and written communication skills, ability to prepare reports, and correspondence to a

high standard for a range of audiences.

• Strong time management skills and ability to work independently, and prioritise tasks

• Proficiency in using computer applications including Microsoft office, client/data management systems

and a willingness to learn new programs

• Demonstrated experience in trauma informed practice or a willingness to engage in training.

• CDP Certificate from DCJ, and / or Knowledge of the Child Protection legislation that underpins the work

of a case manager in Out of Home Care including knowledge of the Office of the Children’s Guardian

NSW Child Safe Standards for Permanent Care and the Children & Young Person (Care and Protection

Act) 1998.

• Ability to undertake the duties of the role in accordance with Family Spirits Mission, Vision and Values

• Demonstrated experience working with CALD and Aboriginal and Torres Strait clients

• Commitment to working within a diverse and inclusive environment

• Current Working with Children Check Clearance

• Nationally Coordinated Criminal History Check (NCCHC)

• Current and valid Driver’s License
