Broome Regional Aboriginal Medical Service (BRAMS) is an Aboriginal Community Controlled Health Service which has been caring for the Broome community for more than 40 years. BRAMS provides comprehensive, holistic, and culturally responsive primary health care and community services to Aboriginal people living in Broome.

In addition to primary health care, BRAMS is an approved aged care and NDIS provider and delivers a range of community services that support the health and wellbeing of Aboriginal people and their families. Services are designed to be person-centred, culturally safe, and responsive to the needs of individuals across the life course.

About the Role

This is an identified position. To be eligible, applicants must identify as Aboriginal and/or Torres Strait Islander, pursuant to Section 50(d) of the Equal Opportunity Act 1984 (WA).

The Aboriginal Health Practitioner delivers high-quality, culturally safe primary health care as part of the multidisciplinary team. The role provides clinical assessments, health checks, wound care, screening, recalls and follow-up, chronic disease support, immunisation support, health promotion and education within AHPRA scope of practice.

You will contribute to care planning, support GPs and nurses in delivering integrated, person-centred care, and provide cultural advice that strengthens engagement with consumers and families. The role works in accordance with the BRAMS Model of Care, Quality Care Framework and Consumer Voice Framework, supporting early intervention and prevention activities.

Key responsibilities include:

• Delivering culturally safe primary health care, including clinical assessments, observations, screening, injections and point-of-care tests within scope.
• Conducting Aboriginal Health Assessments (715s), health checks and screening activities, and contributing to GP Management Plans and Team Care Arrangements.
• Participating in recalls and reminders for health checks, pathology, immunisation and chronic disease reviews.
• Providing wound care, dressings and ongoing monitoring as directed, and supporting immunisation activities including preparation and post-vaccination monitoring.
• Supporting case management and care coordination for consumers with chronic or complex conditions, including outreach and home visits.
• Providing cultural advice to clinical staff to support safe and appropriate communication and care.
• Delivering individual and group education sessions and participating in BRAMS community events and screening activities.
• Documenting all assessments, care and follow-up in MMEx contemporaneously.

About You

To be successful in this role, you will have:

• A Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Practice).
• Experience delivering primary health care or community health services.
• An understanding of chronic disease management and clinical follow-up processes.
• The ability to complete Aboriginal Health Assessments and management plans within scope.
• The ability to work collaboratively with GPs, Registered Nurses, Aboriginal Health Practitioners, allied health and community teams.
• Strong organisational skills and the ability to manage competing priorities.
• Strong communication and cultural engagement skills.
• The ability to deliver culturally safe and trauma-informed care.

Experience in an Aboriginal Community Controlled Health Organisation, experience with GP Management Plans and Team Care Arrangements, and familiarity with primary health care clinical software will be highly regarded.
