About the Role

As a Care Partner, you'll be part of one of our collaborative regional teams, supporting older Australians to remain living safely, independently and with dignity in their own homes.

You'll become a trusted point of contact for your clients, working closely with families, Care Partners, Clinical staff, Allied Health professionals and Rostering teams to coordinate services, solve problems and deliver exceptional person-centred care.

What You'll Be Doing

• Building trusted relationships with clients and their families.
• Coordinating services that support clients to live safely and independently at home.
• Conducting care reviews and developing person-centred care plans.
• Managing client budgets in line with the Support at Home program to maximise outcomes and funding.
• Working collaboratively with Care Partners, Clinical staff, Allied Health professionals and Rostering teams.
• Identifying risks and opportunities to improve client outcomes.
• Advocating for clients and supporting them throughout their aged care journey.
• Managing a varied caseload while delivering exceptional customer service.

Essential Requirements

• Previous experience as a Care Partner
• Current driver’s licence
• National Police Clearance
• Updated influenza vaccination
