About the role
Disability Services Adelaide is a South Australian community services provider committed to delivering high-quality, person-centred support to people living with disability. We work closely with individuals, their families, support workers, and community service providers to coordinate tailored supports that promote independence, inclusion, choice, and overall wellbeing.
About the role:
We are seeking a compassionate, organised, and motivated Client Coordinator to join our growing team. This role is responsible for coordinating services for individuals receiving disability and aged care supports, ensuring clients receive high-quality, person-centred services that meet their individual goals and needs.
The successful applicant will work closely with the team, clients, families, support workers, and external service providers to coordinate and monitor service delivery while maintaining compliance with organisational policies and relevant industry standards.
Key Responsibilities:
· Coordinate and manage community-based support services for clients.
· Support participants to build their capacity to independently coordinate and manage their own supports by providing information, facilitating referrals, and connecting them with informal support networks, community resources, and appropriate service options.Conduct client intake, assessments, and service planning.
· Monitor service delivery and ensure supports are delivered in accordance with client needs and organisational standards
· Liaise with NDIS providers, Home Care providers, allied health professionals, and other community organisations to facilitate appropriate supports.
· Maintain accurate client records, case notes, and documentation.
· Coordinate staff rosters and schedule services to ensure continuity of care.
· Participate in regular client reviews and update support plans where required.
· Ensure compliance with relevant legislation, privacy requirements, and company policies.
· Build positive relationships with clients, families, support workers, and external stakeholders.
Criteria:
· Experience in working with people within the disability sector
· A relevant Diploma or higher qualification is highly desirable
· Demonstrated ability to build positive, empowering, and supportive relationships with clients and their families while effectively responding to individuals with complex support needs.
· Sound understanding of current disability-related legislation, policies, and industry practices.
· Ability to interpret NDIS plans and recommend suitable support services in accordance with participants' goals, plan objectives, and available funding.
· Excellent verbal and written communication skills
· COVID-19 Vaccination
· A current driver’s licence and vehicle (mandatory)

Pay: $80,000.00 – $85,000.00 per year
