About the role
We are seeking a Support Coordinator to join our team in Bankstown, NSW on a part time or full-time basis. The Support Coordinator plays a vital role in our organisation, acting as the primary point of contact for our service users and their families. You will be responsible for coordinating, planning and delivering individualised support services that enable people with disability and aged care needs to achieve their personal goals and maintain their independence. This role is central to ensuring our clients receive seamless, coordinated care that meets their unique requirements and aspirations.
Key responsibilities
Conducting comprehensive assessments of client needs, preferences and goals to develop personalised support plans
Coordinating and arranging appropriate support services and community resources to meet identified client needs
Maintaining regular contact with clients and their families to monitor progress, gather feedback and adjust support arrangements as required
Managing client budgets and ensuring services are delivered within approved funding allocations
Liaising with service providers, healthcare professionals and other stakeholders to ensure integrated and holistic service delivery
Documenting client information, support plans and outcomes in accordance with regulatory requirements and organisational policies
Advocating on behalf of clients to ensure their rights, preferences and dignity are respected and upheld
Attending meetings and reviews with clients, families and relevant professionals to evaluate support effectiveness
Contributing to the continuous improvement of support services through feedback collection and reporting
Maintaining confidentiality and adhering to all relevant legislation, codes of conduct and ethical standards
What we're looking for
Relevant qualification in community services, social work, health or a related discipline, or demonstrated equivalent experience in support coordination or case management
Knowledge of the National Disability Insurance Scheme (NDIS) and/or aged care funding and service systems
Strong communication and interpersonal skills, with the ability to work sensitively with people from diverse backgrounds
Excellent organisational and time management abilities, with capability to manage multiple client cases simultaneously
Demonstrated commitment to person-centred practice and the principles of choice, dignity and respect
Ability to work both independently and collaboratively within a team environment
Strong problem-solving skills and the capacity to exercise professional judgement in complex situations
Proficiency with computer systems and willingness to engage with digital tools and platforms
Current Working with Children Check and NDIS Worker Screening Check or willingness to obtain these
Valid driver's license and access to reliable transport to visit clients as required
