Home Care Manager

Just Better Care Sunshine Coast is looking for an experienced Home Care Manager to lead our Home Care Partners team and drive outstanding participant outcomes under the new Support at Home Program.

This is a hands-on leadership role for someone who thrives on both people management and participant-centred care coordination, someone who can balance compliance, compassion, and commercial results.

About the Role

Reporting to the Franchise Owner, the Home Care Manager oversees a caseload and a team of Home Care Partners, ensuring individualised support plans are developed, reviewed, and delivered in line with each participant's assessed needs. You'll provide day-to-day leadership and supervision of the team, step in on caseload when needed, and ensure every aspect of service delivery meets the Aged Care Quality Standards, the Aged Care Act 2024, and the Support at Home Program requirements.

What You'll Bring

• Qualifications in Community Services, Human Services, Nursing or equivalent
• Working knowledge of the Support at Home Program and Aged Care Quality Standards
• Previous experience in a Case Management role, with proven experience supervising and training staff
• Strong administrative, communication, and problem-solving skills
• Current NDIS Worker Screening Card, Working with Children Check, and Driver's Licence
