Position Title: Clinical Care Partner - Support at Home (Aged Care)

PURPOSE OF THE POSITION

The Clinical Care Partner is responsible for delivering person-centred clinical care coordination and support services to older people receiving care through the Australian Support at Home program.

The role works collaboratively with clients, families, care workers, health professionals, and service providers to ensure clients receive safe, high-quality, and responsive care that supports independence, wellbeing, and quality of life.

The Clinical Care Partner conducts clinical assessments, develops and reviews care plans, coordinates services, monitors clinical risks, and ensures services align with the client's goals, preferences, and the budgeted funding allocation under the Support at Home program.

ATTRIBUTES REQUIRED FOR THIS POSITION

· Registered nurse with APHRA

· Experience with Clinical Case Management

· High-level communication skills both verbal and written

· Demonstrated knowledge of the aged care standards and how they relate to everyday service

· Ability to work collaboratively in a team environment

· Ability to engage with both internal and external stakeholders

KEY RESPONSIBILITIES

Clinical Care Coordination

• Conduct comprehensive clinical and health assessments for clients receiving Support at Home services.
• Develop, implement, monitor, and review individualised care plans based on assessed needs, risks, and personal goals.
• Coordinate clinical and allied health services to support clients to remain living safely and independently at home.
• Monitor changes in client health status and respond appropriately to emerging clinical needs.
• Facilitate referrals to health professionals, community services, and specialist providers as required.
• Support transitions between home care, hospital, residential aged care, and other care settings.
• Ensure that all care needs can be met within the budget
• Conduct nursing care as required

Client Partnership and Advocacy

• Build trusted relationships with clients, families, carers, and representatives.
• Ensure clients are actively involved in decision-making regarding their care and support services.
• Advocate for client rights, choice, dignity, independence, and cultural safety.
• Support informed decision-making regarding available services and funding options.

Risk Management and Clinical Governance

• Identify, assess, and manage clinical risks including falls, medication management, wounds, nutrition, cognition, and chronic disease management.
• Escalate clinical concerns in accordance with organisational policies and procedures.
• Contribute to incident reporting, investigation, and quality improvement activities.
• Ensure clinical documentation is accurate, timely, and compliant with legislative requirements.

Funding and Care Planning

• Support effective utilisation of Support at Home funding in line with assessed needs and program guidelines.
• Monitor service delivery against care plans and budget allocations.
• Collaborate with care management teams to ensure services remain sustainable and responsive.
• Assist clients in understanding the available services and clinical supports within their funding classification.

Quality and Compliance

• Ensure compliance with:
• Support at Home Program requirements.
• Aged Care Act and associated regulations.
• Aged Care Quality Standards.
• Clinical governance frameworks.
• Organizational policies and procedures.

Participate in audits, accreditation activities, and continuous improvement initiatives.

• Timely completion of assessments and care plan reviews.
• Achievement of client-centred care outcomes.
• Effective management of clinical risks.
• Compliance with documentation and reporting requirements.

ESSENTIAL QUALIFICATIONS AND EXPERIENCE

Qualifications

• Current registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse,
• Relevant tertiary qualification in Nursing.

Experience

• Minimum 2–3 years' experience in aged care, community health, home care, or chronic disease management.
• Experience conducting clinical assessments and care planning.
• Experience coordinating services for older people living in the community.
• Knowledge of person-centred and consumer-directed care principles.

Knowledge

• Understanding of the Australian aged care sector and Support at Home reforms.
• Knowledge of Aged Care Quality Standards and clinical governance principles.
• Understanding of chronic disease management, dementia care, falls prevention, medication management, ventilator & oxygen management and care coordination.
• Knowledge of safeguarding and elder abuse prevention principles.

MANDATORY REQUIREMENTS

• Current AHPRA registration (where applicable).
• Current National Police Check.
• NDIS Worker Screening Check (if required).
• NDIS Orientation Module
• Australian Working Rights
• Current Driver's Licence.
• Evidence of required vaccinations in accordance with organizational policy and public health requirements.

PAY RATE

Registered Nurse (RN) Level 2 – Nurses Award. Salary is based on experience, starting from $47.34 per hour plus superannuation. Award wage increases effective 1 July 2026 will be applied in accordance with the Nurses Award.
