NDIS Support Coordination

We are seeking an experienced and motivated NDIS Support Coordinator to join our growing team. This role is ideal for someone with a strong background in case management who is passionate about supporting people with disability to navigate the NDIS and achieve their goals.

As a Support Coordinator, you will work closely with NDIS participants, their families, and service providers to ensure participants receive the right supports, build capacity, and maintain choice and control.

Key Responsibilities

• Provide Support Coordination in line with NDIS guidelines
• Assist participants to understand and implement their NDIS plans
• Coordinate and link participants with appropriate services and supports
• Undertake comprehensive assessments, goal planning, and risk management
• Monitor plan utilisation and support participants through reviews
• Maintain accurate case notes, reports, and documentation
• Advocate for participants to ensure their needs and goals are met
• Build and maintain strong relationships with participants, families, and stakeholders

Essential Criteria

• Minimum 1–2 years’ experience in case management (disability, community services, mental health, or related field)
• Sound understanding of the NDIS framework and service system
• Strong organisational and time management skills
• Excellent written and verbal communication skills
• Ability to work autonomously and manage a varied caseload
• High level of professionalism, empathy, and problem-solving ability
• NDIS Worker Screening Check (or willingness to obtain)
• Working with Children Check (if applicable)
• Current Australian Driver’s Licence and access to a vehicle

Desirable Criteria

• Experience as an NDIS Support Coordinator or Specialist Support Coordinator
• Relevant qualification (Community Services, Social Work, Psychology, Disability, or similar)
• Experience working with complex needs and multiple stakeholders
