Support Coordinator

As part of the Access Support Coordination team in Spring Hill, you'll be the person participants lean on when navigating the NDIS feels overwhelming. You'll help them understand their options, make informed decisions and get every bit of value out of their funding — all while building the kind of trust that makes a real difference in someone's life.

What You'll Be Doing

• Supporting participants to build meaningful, connected lives beyond their funded supports
• Helping participants understand and get the most out of their NDIS plans
• Coordinating supports efficiently to maximise outcomes and funding utilisation
• Gathering supporting evidence that reflects participants' evolving needs and future funding requirements
• Identifying opportunities across employment, recreation, education and housing
• Advocating for participants and liaising with mainstream services to ensure the right supports are in place

You're the Right Person If

• You have proven experience in a Support Coordination role within the disability sector
• You hold qualifications in Disability, Community Services, Social Work or a related field
• You have a strong knowledge of the NDIS and its processes
• You're able to manage KPIs and billable hour targets effectively
• You're a natural connector, excellent with people and confident solving problems with a genuine participant-first approach
• You hold a current Driver's Licence, Working with Children Check and NDIS Worker Screening Clearance
