Community Assessment & Care Finder Officer

Employment Status: Parental Leave Cover (Fixed term until September 2027)
Days & Hours of Work: 3 days (days to be negotiated) 22.5 hours per week
Competitive Salary: Band 6 - $101,833.74 pro rata + 12% superannuation
Location: Council office in Sandringham and in the field with minimal opportunity to work from home

About the Role:

The Care Finder and Community Assessment Officer supports older people, particularly those experiencing homelessness, housing insecurity, or vulnerability, to navigate the aged care system and access support services that promote safety, wellbeing, and independent living.

Key Selection Criteria:

To be successful in this role, you will have:

• A tertiary qualification in a health, allied health or social science discipline (Bachelor Level).
• Minimum two years' experience working with older people who are experiencing vulnerability and/or homeless, or at risk of homelessness as well as assisting people of all ages with a disability.
• Experience within the mental health sector would be well regarded.
• A comprehensive understanding of My Aged Care, the Care Finder Program, Commonwealth Home Support Program, Support at Home Program, Single Assessment services and Home and Community Care for Younger People Program (HACC PYP).
• Excellent written and verbal communication skills including an ability to communicate with people from diverse backgrounds.
• An ability to identify client needs and respond appropriately in an empathetic, objective, respectful and professional manner.
• The ability to work as part of a team, and a strong commitment to maintaining a high standard customer service culture.

Mandatory pre-employment requirements:

• Be willing to undergo and complete a satisfactory criminal history check and ongoing checks every three years.
• Provide or obtain an employment-based Working with Children check. 
• Valid Victorian Driver’s Licence.
