Location: Council office in Sandringham and in the field with minimal opportunity to work from home

About the Role:
The Care Finder and Community Assessment Officer supports older people, particularly those experiencing homelessness, housing insecurity, or vulnerability, to navigate the aged care system and access support services that promote safety, wellbeing, and independent living.
