Hospital in the Home (HITH) is a contemporary healthcare service that enables patients to receive acute and subacute hospital-level treatment in the comfort of their own homes. Based at Cumberland Hospital, you will be part of a highly skilled multidisciplinary team committed to delivering safe, evidence-based and patient-focused care across Western Sydney.

What you'll be doing
As a Registered Nurse within Hospital in the Home, you will provide high-quality, evidence-informed nursing care that promotes positive health outcomes and supports patients throughout their healthcare journey. You will work autonomously while collaborating closely with patients, carers and healthcare professionals to deliver safe and effective care in community settings.
· Assess, plan, implement and evaluate person-centred nursing care in accordance with professional standards and best practice
· Deliver timely, culturally appropriate care to patients with a variety of physical and mental health needs
· Maintain accurate clinical documentation, care plans, progress notes and patient records
· Collaborate effectively with patients, carers and multidisciplinary teams to achieve optimal care outcomes
· Support quality improvement initiatives, evidence-based practice and the development of nursing colleagues and students

Essential Criteria:
· Registration: Currently authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (APHRA)
Selection Criteria:
1. Current unrestricted Drivers License
2. Recent relevant experience in clinical setting as deemed appropriate by WSLHD
3. Excellent time management and organisation skills with attention to detail
4. Evidence of computer literacy and understanding of relevant health related information systems
5. Effective interpersonal skills with a demonstrated ability to develop and maintain relationships with clients, carers and the wider health care team
