Central Coast Local Health District (CCLHD) has an opening for permanent full-time Registered Nurse to join the Hospital in the Home (HITH) team at Gosford, Wyong Hospitals, and the Long Jetty Health Care facility. In this vital role, you will serve as primary health care worker in the community, providing acute clinical care to patients in their homes or at HITH clinics. You will collaborate with multidisciplinary teams to monitor health statuses, respond to episodic health needs, and promote health maintenance, rehabilitation, and disease prevention.
This is a targeted recruitment under Rule 26 of the Government Sector Employment (General) Rules 2014 where only Aboriginal or Torres Strait Islander people are considered eligible to apply for this role. Aboriginal or Torres Strait Islander applicants must demonstrate Aboriginal or Torres Strait Islander status in addition to addressing the selection criteria.
About the Role
As a Registered Nurse in the HITH program, your primary responsibilities will include delivering acute clinical care to patients in their place of residence or at a HITH clinic, working in close collaboration with the multidisciplinary HITH team. Close ties with both the community sector and the acute hospitals within Central Coast Local Health District will be required. As the settings in which care is delivered, HITH nurses require skilful application and adaptation of evidence-based practice on which nursing is based.
You will play a key role in driving innovation and leading the development and delivery of patient education programs, ensuring that patients and their families are equipped with the knowledge, skills, and resources needed to make informed healthcare decisions. This includes providing education on their conditions, medications, and self-care strategies, enabling them to actively participate in managing their health and improving their overall wellbeing.
Flexibility and adaptability are key, as you will need to assess and respond to a variety of clinical situations in diverse home settings. You will actively promote health, recognise early signs of disease, and provide timely interventions to help patients maintain their independence and well-being. Additionally, you will record data for service evaluation and contribute to continuous quality improvement efforts within the team.
