Clinical Care Coordinator - Flora MacDonald
About The Role
This position is a 6-month fixed-term contract, with the scope to extend and potentially convert to a permanent full-time position.
As the Clinical Care Coordinator, you are responsible for providing clinical leadership to staff and ensuring compliance with legislative requirements to enable us to continue to deliver a high standard of care, services and most importantly a quality lifestyle for our residents.
Key Responsibilities
Coordinate the delivery of quality clinical services in line with a resident centred model of care Manage and provide efficient, effective and compassionate care planning and assessmentResponsible to ensure medication management is provided in a safe and effective manner in line with legislative requirements and policyWork collaboratively with staff, other health professionals, the resident, their families and Clinical Leadership teams to ensure clinical decisions are based on best practice and positive resident outcomes
What You Bring
Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (APHRA) (mandatory)Demonstrated knowledge and understanding of the legislation governing aged careClinical Care Coordinator experience is requiredCommitment to contemporary aged care practiceAbility to support and lead a multi-disciplinary teamExperience in a clinical leadership role with the ability to manage day to day clinical issuesExcellent oral and written communication skillsAbility to plan, prioritise and manage day to day activities in an efficient mannerDemonstrated computer literacyA desire to join a team committed to the care of others in a mission based organisation
