Support Coordinator

Are you passionate about empowering people with disability to achieve their goals and live the life they choose? We’re looking for a compassionate, organised and motivated Support Coordinator to join our team. This position is a 12 month, part time contract covering 60 hours a fortnight. The role will be based at our Newmarket office.

About the role:

In this rewarding role, you’ll work alongside NDIS participants to help them understand and implement their NDIS plans, connect with the right supports and services, and build the confidence and skills to increase their independence. You’ll be part of a collaborative team that values person-centred practice, inclusion and positive outcomes.

As a Support Coordinator, you will:

• Support participants to understand and implement their NDIS plans in line with their goals.
• Coordinate funded, mainstream and community supports that meet participants’ individual needs.
• Build strong relationships with participants, families, service providers and community organisations.
• Assist participants to make informed decisions while promoting choice, control and independence.
• Monitor participant progress and funding utilisation, identifying any risks or barriers to achieving outcomes.
• Prepare participants for NDIS plan reviews by gathering relevant information and documentation.
• Maintain accurate case notes, records and billable activities in accordance with organisational and NDIS requirements.
• Escalate complex participant matters to the Support Coordination Team Leader when required.
• Contribute to a positive, collaborative and participant-focused team culture.

About you:

You’ll be someone who enjoys building relationships, solving problems and supporting people to achieve meaningful outcomes. To be successful in this role, you will have:

• Experience in Support Coordination, case management, disability, community services or a related field.
• A sound understanding of the NDIS and person-centred practice.
• Excellent communication and interpersonal skills with the ability to build trust and rapport.
• Strong organisational and time management skills, with the ability to manage competing priorities.
• Experience maintaining accurate records and working with digital client management systems.
• The ability to work independently while collaborating effectively with a multidisciplinary team.
• Sound judgement, professional boundaries and a commitment to participant safety and wellbeing.

Essential requirements:

• Relevant qualification in Community Services, Disability, Social Work, Human Services or a related discipline (or equivalent experience).
• Current Driver Licence and access to a reliable vehicle.
• Current NDIS Worker Screening Check (or ability to obtain).
• Current Working with Children Check (where applicable).
• Current First Aid and CPR Certificate (or willingness to obtain).
