Make a meaningful difference in the lives of people across the Cairns region by supporting access to high-quality, culturally responsive allied health care. As a Community Clinical Assistant (Identified), you will work within a multidisciplinary team to assist with delegated clinical and non-clinical activities across a range of community programs, helping deliver safe, effective care that improves health, wellbeing and independence for consumers, families and communities.About us:
The Adult Community Health Service delivers multidisciplinary, person-centred care through a range of community-based programs, including Care Coordination, Community Rehabilitation, Post-Acute Care, Falls services, Pulmonary Rehabilitation, Persistent Pain Management, Refugee Health, and Aboriginal and Torres Strait Islander Health Worker services. Delivered across the Cairns region through clinic, home, outreach, and community-based models, the service brings together nursing, allied health, Aboriginal and Torres Strait Islander Health Workers, and support staff to work in partnership with consumers, families, carers, healthcare providers, and community organisations to improve health and wellbeing, promote functional independence, support timely discharge and integrated care, and reduce avoidable hospital presentations.
